To activate Hex Pentimento, the killer then has to activate the broken Totem to replace it with the Hex. You can do this with up to 4 broken totems for stacking effects (assuming the survivors actually break all 5). If no totems are ever cleansed, then Pentimento will never be able to activate and is 100% useless.
